Insurances Accepted
Family Practice Medical Center accepts the following types of health plans/insurance:
Payment is due at the time of service. We accept Visa, MasterCard and Discover, as well as personal checks and cash. If your visit is covered by health insurance we can file your insurance claim if you bring your plans membership card. Co-pays are to be paid at the time of service. We accept most major insurances, including but not limited to, CIGNA, Health Alliance, Blue Cross/Blue Shield, United Healthcare, Medicare, Medicaid, Personal Care, Wellcare, and Humana. If you don’t see your insurance listed please feel free to call either of our offices and ask if we accept it.
If you are planning on seeing us as a new patient feel free to print out these forms and bring them filled out to our office in order to speed up the new patient process.
